Congratulations on taking this step toward better dental health. We diagnose and treat oral pathology, biting discrepancies, and TMJ dysfunction (Temporomandibular Joint Dysfunction). We work closely as a team with your referring dentists. We are extraordinarily proud of our staff, which is comprised of caring and dedicated professionals. They communicate well with patients and each other, ensuring that the patient’s surgical and administrative needs are met as efficiently and smoothly as possible.
Our goal is to make you feel comfortable and relaxed. We are here to listen to your questions and concerns and to help provide solutions leading to your better health. Please don’t hesitate to email us, info@kellerperio.com if you have any questions regarding your care, or call us at (302) 652 3586.
Your initial appointment will consist of a consultation addressing your chief complaint and treatment options.
Please assist us by providing the following information at the time of your consultation:
Important: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays: If your dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
Please be advised we are not a contracted provider with any insurance plan or company. We will be happy to assist you with the filing of all your insurance claims to help you receive the maximum reimbursement. It is important for you to remember that your dental insurance contract is between you and your employer. Therefore this contract is not with our office and we cannot guarantee your coverage or benefits. At your request we will be happy to send a claim for a pre-determination of dental benefits for any proposed treatment.
Our goal is to provide you with the highest quality of dental services using the latest techniques and technology. One way we accomplish this is by eliminating costly billing procedures and requesting payment at the time of your visit. We are sensitive to the fact that everyone has different needs and we provide the following payment options:
Our office accepts cash, personal checks, certified checks and money orders.
We accept Visa, Master Card, American Express, Discover
We offer financing through CARE CREDIT for qualified individuals. Ask one of our Administrative Assistants at our reception desk for details. Your company may offer a dental spending account or a ’flexible’ spending account, which allows you to set aside pre-tax dollars to pay for your dental expenses. Please contact your employer for details.
