Your initial appointment will consist of a consultation addressing your chief complaint and treatment options.

Important: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays: If your dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
Please be advised we are not a contracted provider with any insurance plan or company. We will be happy to assist you with the filing of all your insurance claims to help you receive the maximum reimbursement. It is important for you to remember that your dental insurance contract is between you and your employer. Therefore this contract is not with our office and we cannot guarantee your coverage or benefits. At your request we will be happy to send a claim for a pre-determination of dental benefits for any proposed treatment.

We offer financing through CARE CREDIT for qualified individuals. Ask one of our Administrative Assistants at our reception desk for details. Your company may offer a dental spending account or a ’flexible’ spending account, which allows you to set aside pre-tax dollars to pay for your dental expenses.
